Child Death Overview Panel

Child Death Overview Panels (CDOPs) have a responsibility to review all child deaths (from birth up until 18 years of age and excluding stillborn babies and planned legal terminations), normally resident in their area, as described in Working Together 2018 and the Child death review: statutory and operational guidance.

The Hampshire and Isle of Wight Child Death Overview Panel (CDOP) arrangements document sets out the arrangements for the review of all child deaths, to take place across the Hampshire, Isle of Wight, Portsmouth and Southampton areas.

The CDOP has representatives from Public Health, NHS Clinical Commissioning Groups, Paediatric Departments, Hampshire Constabulary, Children’s Services, Education, Child and Adolescent Mental Health Services (CAMHS), Midwifery Services, School Nursing and Health Visiting providers. Other professionals may be invited to give specialist advice when required.

CDOP collects and reviews information about each child death. Each death is discussed anonymously at bi-monthly CDOP meetings with a view to identifying:

  • Any case giving rise to the need for a review mentioned in Regulation 5 (1) (e) of the Local Safeguarding Children Partnership Regulations 2006
  • Any matters of concern affecting the safety and welfare of children in the area of the authority
  • Any wider public health or safety concerns arising from a particular death or from a pattern in that area

CDOP produce an annual report detailing any recommendations and lessons learnt during the previous year. The annual report is a public document and; therefore, it does not contain information that could identify an individual child or his/her family.

The last 4LSCB CDOP Annual Report 2018-19 is now available. A brief two-page summary of the report has also been published.

CDOP Forms

All organisations working with the child are required to notify CDOP of a child’s death using an agreed template (Child Death Notification Form). A Child Death Reporting Form must also be completed to provide further information. The form templates can be accessed from the government website.

Prevention and Postvention Protocol for Hampshire Schools and Colleges

This protocol has been developed to help schools and colleges in Hampshire identify and support students who may be suicidal and importantly to consider a wide range of associated issues, in the case of a death by (suspected) suicide. It both supports schools/colleges to prepare in case there ever is a (suspected) suicide, and also how to best respond when a suspected suicide has taken place. This protocol will also be developed to support workplaces and communities affected by suicide.
